Male Hair Loss: The Science Behind It and How to Treat It Effectively

Hair loss affects roughly two-thirds of men by age 35, and nearly 85% by age 50. Despite being one of the most common health concerns men face, it remains one of the most poorly understood and inconsistently treated. At HolaVida Health, we take a clinical, evidence-based approach to male hair loss — because the right treatment, started early, can make a meaningful difference.

Understanding male pattern hair loss

Male pattern hair loss — clinically known as androgenetic alopecia — is the most common form of hair loss in men. It follows a predictable progression: a receding hairline, thinning at the crown, and eventual loss across the top of the scalp. The pattern is largely genetic, but the mechanism is hormonal. The primary driver is dihydrotestosterone (DHT), a derivative of testosterone. DHT binds to receptors in genetically susceptible hair follicles, causing them to shrink over time — a process called miniaturization. As follicles miniaturize, they produce thinner, shorter hairs, and eventually stop producing hair altogether.

Why genetics aren't the full story

While genetics determine susceptibility, they don't determine outcome. Two men with identical genetic profiles can have very different hair loss trajectories depending on when they start treatment, how consistently they treat, and what therapies they use. Hair loss is often manageable — and early treatment typically produces the best results. Several factors influence DHT sensitivity, including genetic predisposition (certain androgen receptor variants), testosterone levels, age (DHT accumulates and follicle sensitivity increases over time), and scalp inflammation, which can accelerate follicle miniaturization.

Clinically proven treatments for male hair loss

Finasteride is an FDA-approved oral medication that blocks the enzyme (5-alpha reductase) that converts testosterone into DHT. By reducing DHT levels at the follicle, it can slow or halt hair loss progression — and in many men, stimulate some regrowth. Minoxidil — available topically and in oral form — is an FDA-approved treatment that prolongs the hair growth phase and improves blood flow to follicles, working independently of DHT and often used alongside finasteride. The most effective plans typically combine finasteride and minoxidil, targeting both DHT reduction and follicle stimulation simultaneously — a well-studied approach generally more effective than either treatment alone.

The importance of starting early

Hair follicles that have been dormant for an extended period are much harder to reactivate than those still producing hair. Starting treatment before visible loss is advanced gives providers more to work with and patients better long-term outcomes. Most men wait too long.
